Bill Overview

You have received a Patient Statement because we have performed laboratory services for you at the request of your physician and you have received an Explanation of Benefits (EOB) from your insurance company which indicates the charges for those services and what they have paid us. The EOB indicates the amounts for which you are personally responsible and it is this amount that we require you to pay us directly. It may also be that you have no insurance and are personally responsible for payment.  Please contact our Patient Relations department at 1-855-251-3331 for further information and to discuss financial assistance options which include Prompt Pay Discounts, Uninsured Discounts, Financial Hardship Discounts, and Payment Plans.
